Please print and fill out the appropriate forms. You should bring in a total of 4 pages; the patient registration form (2 pages), and the patient history forms (2 pages).
If you are a minor, you also need to print and bring in the minor consent form.
You do not need to print a copy of the Privacy Practice, as these forms are available in the clinic.
